Dementia Patients Going Missing - Herbert Protocol
The following link is a form which can be completed by the patient/family of someone who has dementia.
This is to help the police find the patient if they go missing.

Memory Support Advisors working with Dementia Forward.
If you are concerned about your memory or the memory of a loved one, the memory support advisors can arrange to see you and have a chat. Patients and relatives can self-refer via the GP practice or be referred by practice staff. They can contact each person using their preferred contact number and arrange a home visit or a conversation on the phone.
They can also pass referrals onto Dementia Forward, with patient's consent, for ongoing support through the diagnostic process. They can also signpost patient and families to relevant services.
