To register with the practice as a new patient please either do this online using the following link https://gp-registration.nhs.uk/B82067 or call into the surgery to collect a registration pack or you can download the forms below to complete and return to us. It would help us with your registration if all adult patients produce proof of identification although this is not a requirement. You can check your postcode here to see if you are within our catchment area.

All new patients are offered a new patient check with a nurse/healthcare assistant. Our reception team will offer this appointment for you when you register. This permits us to meet you, assess and discuss your health needs. If you are taking repeat medication, please ensure that you book an appointment to see one of our doctors before your last prescription runs out. You can choose to see any of our doctors for your day to day needs. However, we encourage you to see one doctor for long term problems, as this ensures continuity of care.
