Power of attorney are to enable an individual to nominate somebody who knows them who could take decisions on their behalf regarding finances and health if that individual becomes unable to makes those decision themselves.
Power of attorney are available in 2 types (financial and health). A link to the relevant forms on the government website can be found here:

You do not need a Doctor to do this, a friend who has known you for 2 years can witness this. The forms can then be registered with the court of protection.
