How to Register

To register as a new patient please either call into the surgery to collect a registration pack or download a copy here, complete it and return it to us. It would help us with your registration if all adult patients produce proof of identification although this is not a requirement.

When you register with us, we will also ask you to complete all of the following documents. These are all available in our new patient pack or can be downloaded below.

The Electronic Prescription Service

All new patients are offered a new patient check with a nurse/healthcare assistant. Our reception team will offer this appointment for you when you register. This permits us to meet you, assess and discuss your health needs. If you are taking repeat medication, please ensure that you book an appointment to see one of our doctors before your last prescription runs out. You can choose to see any of our doctors for your day to day needs. However, we encourage you to see one doctor for long term problems, as this ensures continuity of care.
